
Health Form Policy
At Pediatric Partners, we understand that health forms are an important part of school, daycare, sports, camps, college, and other activities. Our goal is to complete forms accurately and as quickly as possible while ensuring they reflect your child's current health.
Many forms can be completed during your child's annual well visit, helping you avoid delays and additional fees.
School and Daycare Forms
For your convenience, we automatically upload the following documents to your child's patient portal after eligible visits:
- Updated immunization records whenever vaccines are administered
- Well visit forms following annual wellness examinations
Because these forms are generated directly from our electronic health record (EHR), families generally do not need to complete any paperwork in advance for routine Kansas school and daycare forms.
Forms That Require Parent or Patient Information
Some forms require information that only a parent, guardian, or patient can provide. These forms must be completed by you before we can complete and sign the medical portions.
Examples include:
- College health forms
- Family and Medical Leave Act (FMLA) forms
- Missouri school and daycare forms
- School medication administration forms
- Women, Infants and Children (WIC) forms
- Scout forms
- Club sports forms
- Kansas State High School Activities Association (KSHSAA) forms
- Missouri State High School Activities Association (MSHSAA) forms
- Other non-routine Kansas school forms
If you are unsure whether your form requires your portion to be completed first, please ask our staff.
Form Fees
Most health forms are completed at no charge when they are requested during a scheduled annual well visit.
There is no charge for:
- Most forms completed during a well visit
- WIC forms
- Forms for patients covered by Medicaid
Fees may apply when:
- Forms are requested outside of a well visit
- Additional clinician review or documentation is required
- Same-day completion is requested
Because fees are based on the amount of clinician time required, our front office staff cannot reduce, waive, or modify these charges.
Please note that insurance companies generally do not cover fees for completing forms.
What to Expect When Requesting Forms
Complete your portion first
Except for forms automatically generated by our electronic health record, all patient and parent sections must be completed before we can complete the medical portions.
Please allow adequate processing time
We make every effort to complete forms promptly. However, during busy times of the year—especially before school starts—we may receive hundreds of forms each week.
To help avoid delays:
- Submit forms as early as possible.
- Don't wait until the deadline.
- Whenever possible, bring forms to your child's annual well visit.
Completed forms
Once completed, forms will be uploaded to your patient portal, where you can:
- Download them
- Print them
- Share them electronically as needed
Well Visit Requirement
- To ensure that forms accurately reflect your child's current health, we require that medical information used to complete forms be based on a comprehensive well examination performed within the previous 12 months.
- If your child has not had a well visit within the past year, a well visit must be scheduled before most forms can be completed.
Important
Only well visits performed by Pediatric Partners physicians or nurse practitioners may be used to complete forms.
- Physical examinations completed by another office cannot be used for us to sign forms.
- Sick visits, urgent care visits, and problem-focused appointments do not count as annual well visits for form completion.
Guardianship Forms
Guardianship paperwork requires additional documentation.
To complete these forms, we require:
- A recent neuropsychological evaluation
- A current school Individualized Education Program (IEP), if applicable
Helpful Tips
- Keep your child's annual well visit up to date.
- Bring forms to the well visit whenever possible.
- Complete all parent sections before submitting forms.
- Allow extra time during the summer and back-to-school season.
- Check your patient portal first—your child's immunization record and well visit form may already be available.
